Inclusion criteria

Inclusion criteria: 1. Adults aged 18 years or older. 2. Diagnosed with heart failure with left ventricular ejection fraction below 50 percent confirmed by echocardiography. 3. Hospitalized for acute heart failure and discharged in stable condition. 4. Receiving guideline-directed medical therapy for heart failure. 5. Own and are able to use a smartphone and the electronic health monitoring application. 6. Able to communicate and complete study questionnaires. 7. Provide written informed consent.

Exclusion criteria

Exclusion criteria: 1. Severe cognitive impairment, psychiatric disorders, impaired consciousness, or other conditions that prevent reliable communication or completion of study procedures. 2. Severe hearing or visual impairment that prevents use of the electronic health monitoring application. 3. Terminal illness or other severe comorbidities with an expected life expectancy of less than six months. 4. End-stage renal disease requiring dialysis or other serious medical conditions that would interfere with study participation or follow-up. 5. Inability or unwillingness to use a smartphone or the electronic health monitoring application. 6. Refusal or inability to provide written informed consent.
