Inclusion criteria
Inclusion criteria: 1.Patients aged 18 years or older who have been diagnosed with a first ever stroke. 2.Patients receiving continuous follow-up care at the Outpatient Department (OPD). 3.Patients with stable vital signs and a Modified Rankin Scale score of 0 to 2 (indicating good functional independence). 4.Cognitive function regarding orientation to person, time, and place assessed using the Chula Mental Test (CMT), with a score of greater than or equal to 15 points. 5.Patients who can communicate effectively in Thai, understand the study information, and provide written informed consent by themselves. 6.Patients who have a mobile phone and are able to communicate via the Line application.
Exclusion criteria
Exclusion criteria: 1.Patients with severe cognitive impairment, dementia, or inability to communicate. 2.Patients with severe comorbid complications, such as end-stage heart failure or advanced-stage cancer.
