Inclusion criteria
Inclusion criteria: 1.Have discharge planning and order from physicians for discharge to home, 2. Have NYHA class I to III, stable vital signs without inotropic drug, 3. Able to listen, read, and speak Thai language, 4. Have normal cognitive function, patients who are age equal and more than 60 years old have a Mini-cog assessment score of 3 or higher, 5. Have their own smartphone with internet, or, if the patients do not have a smartphone, a family caregiver must have a smartphone with internet access, 6. Be able to use the Line application independently, or, if the patients do not have the ability to use the LINE application, a family caregiver who is capable of using the application may use it on either the patients or the family caregivers smartphone, provided that only one device is used, 7. Have family caregivers involve in receiving information and collaborating in the development of the treatment plan and individualized care plan and attend sessions on individual care plan monitoring guidelines and utilize the Line application.
Exclusion criteria
Exclusion criteria: 1. Have been diagnosed with psychiatric disorder such as Depression, Anxiety, Schizophrenia from hospitals medical record or family caregiver has psychiatric disorder, 2. Have been diagnosed with neurological disorders such as Dementia, Alzheimer from hospitals medical record, 3. Bedridden or disabilities, 4. Have been diagnosed with terminal stage illness such as advance stage of HF, palliative care, or end of life from hospitals medical records.
