Inclusion criteria

Inclusion criteria: 1. Age of 18 years or older and voluntarily consenting to participate in the study, 2. An HbA1c level greater than or equal to 7.0 (indicative of poorly controlled T2DM), 3. Ability to communicate coherently (e.g., no severe cognitive impairment affecting communication), 4. Access to a mobile phone or smartphone for telehealth monitoring, or reachable by telephone, 5. Continuous receipt of care at the clinic for at least the preceding six months without missing appointments, and 6. No severe acute exacerbation of comorbidities or complications requiring hospitalization at the time of recruitment (e.g., ketoacidosis, severe hypoglycemia).

Exclusion criteria

Exclusion criteria: 1. Presence of severe comorbid conditions requiring hospitalization at baseline, 2. Inability to participate in the study procedures, and 3. Planned relocation during the study.
