Inclusion criteria

Inclusion criteria: 1. Diagnosed with coronary artery disease (CAD) by a physician. 2. Living in a semi-rural community in Nakhon Pathom province. 3. Possessing a smartphone and capable of using mobile applications. 4. Willing to participate in the 12-week intervention.

Exclusion criteria

Exclusion criteria: 1. Patients with severe complications or unstable physical conditions that prevent study participation. 2. Patients with cognitive impairment or inability to communicate effectively. 3. Patients who planned to move out of the study area during the 12-week period.
