Inclusion criteria

Inclusion criteria: 1. age between 35 and 65 years 2. Hemoglobin A1c (HbA1c) level of 7.0 or higher 3. Duration of diabetes of at least 1 year 4. ability to communicate via telephone or smartphone

Exclusion criteria

Exclusion criteria: Patients with severe medical conditions affecting glycemic control or those unable to participate in telehealth communication
