Conditions

Malocclusion requiring orthodontic treatment and therapeutic dental extractions in adolescent patients. This includes moderate to severe dental crowding, bimaxillary protrusion, and dental protrusion requiring space closure, anchorage control, and root alignment using clear aligners or fixed orthodontic appliances. Interventions

Adolescent patients requiring orthodontic extractions will be treated using Invisalign clear aligners according to an individualized digital treatment plan. Aligners will be worn twenty two hours per

Inclusion criteria

Inclusion criteria: 1 Diagnosis of dental malocclusion requiring therapeutic premolar extractions as part of the orthodontic treatment plan due to severe crowding or dental protrusion. 2 Presence of full permanent dentition from first molar to first molar in both upper and lower arches, excluding third molars. 3 Complete baseline diagnostic records available at the start of treatment, including high-resolution digital intraoral scans, panoramic radiographs, and lateral cephalometric radiographs. 4 Patient willingness and commitment to comply with the prescribed treatment protocol, including the required daily wear-time instructions and scheduled follow-up appointments. 5 Patient agreement to complete all standardized patient-reported outcome questionnaires regarding oral health quality of life, comfort, and satisfaction at the designated intervals. 6 Provision of written informed consent by a parent or legal guardian and written assent by the adolescent participant prior to initiating any study proc

Exclusion criteria

Exclusion criteria: 1 Presence of craniofacial anomalies, cleft lip or palate, or other syndromic conditions affecting dental and skeletal development. 2 Severe skeletal discrepancies requiring combined orthodontic and orthognathic surgical intervention. 3 Pre-existing active periodontal disease, extensive untreated dental caries, poor oral hygiene, or root resorption detected on baseline radiographs. 4 Medical history of chronic systemic conditions affecting bone metabolism or current use of medications known to alter bone turnover rates such as chronic corticosteroids or non-steroidal anti-inflammatory drugs. 5 Previous comprehensive orthodontic or orthopedic treatment that could alter baseline dental arches or root positions. 6 Inability to complete the required follow-up appointments, digital scans, or questionnaires due to logistical constraints or non-compliance.
