Inclusion criteria
Inclusion criteria: Participants were eligible if they met either of the following criteria: 1. Older adults with Activities of Daily Living (ADL) scores ranging from 0 to 11 (homebound or bedridden); or 2. Older adults with non-communicable diseases (NCDs) and ADL scores ranging from 12 to 20 who required close supervision of care.
Exclusion criteria
Exclusion criteria: 1. Acute medical conditions requiring immediate hospitalization at baseline 2. Terminal illness with expected survival less than 3 months 3. Inability to be followed up at home due to institutionalization or absence of caregivers.
