Inclusion criteria

Inclusion criteria: 1. Diagnosed with and treated for acute coronary syndrome. 2. Able to communicate in Thai language. 3. Having normal cognitive function ; participants aged sixty years or older must have a GP COG score of nine. 4. New York Heart Association functional class Ito III. 5. Patients or caregivers are able to communicate via smartphone; if patients have limitations in smartphone use, a caregiver who can communicate via smartphone is required.

Exclusion criteria

Exclusion criteria: 1. Physical disability or mobility limitation affecting walking or movement, assessed before hospital discharge.

MeasureTime frame
Recurrent Cardiovascular Risk Recurrent cardiovascular risk will be assessed twice using clinical data and lipid profile results at baseline and 4 to 6 weeks after discharge. U-Prevent (www.u-prevent.com)
