Periodontal disease is a significant cause of losing teeth in adult especially DM patient. Early prevention of periodontitis (appropriate self care ,managing risk factors and oral behavioral modification) can prevent and control periodontal disease for adults .The 3-self (Self efficacy,Self regulation,Self care) behavior modification concept using the Promise Model has been researched and proven to be effective in modifying sustainable health behaviors, emphasizing the motivation of individuals

Inclusion criteria
Inclusion criteria: 1. DM adult 2.aged 30-59 years old 3.has at least 4 usable teeth left. 4.no psychological or neurological disease that affects the perception or compliance with the program.
Exclusion criteria
Exclusion criteria: 1.People with serious underlying medical conditions, such as critical heart disease, end-stage renal disease, cancer under treatment, or severe oral health problems. 2.People who regularly smoke or drink large amounts of alcohol, 3.pregnant women
