Adults (age over 18 years old) with acute respiratory failure from any cause High-flow nasal cannula, IVC diameter, IVC collapsibility index, Point-of-care ultrasound OR POCUS,

Interventions

Patients with respiratory failure receiving high-flow nasal cannula (HFNC) therapy at different flow rates (30, 40, 50, and 60 L/min).

Inclusion criteria

Inclusion criteria: 1.Age more than 18 years old, 2.Patients diagnosed with acute respiratory failure or shock (defined as systolic blood pressure below 90 mmHg or mean arterial pressure below 65 mmHg, accompanied by signs of inadequate peripheral perfusion such as cold extremities), from any cause

Exclusion criteria

Exclusion criteria: 1.Pregnancy, 2.Patients who require immediate endotracheal intubation or non-invasive ventilation (NIV) upon arrival to the emergency department (such as decreased level of consciousness, coma, or severe air hunger) Patients with severe hypoxemic respiratory failure , 3.Patients with life-threatening cardiac arrhythmias (such as sinus node dysfunction or atrioventricular block with heart rate below 50 times per minutes, supraventricular tachycardia , or ventricular tachycardia ), 4.Patients with acute coronary syndrome that need emergency intervention, 5.Situations in which performing ultrasound would delay essential treatment (such as patients requiring urgent endotracheal intubation) Patients with known pulmonary hypertension, tricuspid regurgitation (TR), cardiac tamponade, pulmonary embolism, or acute respiratory distress syndrome (ARDS), 6.Patients with cardiac arrest, 7.Patients unable to tolerate the supine position for ultrasound examination.
