Eligibility
Inclusion criteria
Inclusion criteria: - Patients aged between 18 and 70 years. - Patients diagnosed with secondary substernal goiter based on preliminary physical examination (i.e., the lower border of the mass cannot be palpated even with full neck extension) or radiographic findings suspicious of intrathoracic thyroid extension. - Patients who have indications for and intend to undergo thyroid surgery (total thyroidectomy, lobectomy, or subtotal thyroidectomy).
Exclusion criteria
Exclusion criteria: - Patients with contraindications to computed tomography (CT) scan (e.g., history of severe contrast media allergy, pregnancy). - Patients who cannot receive contrast media due to abnormal renal function. - Diabetic patients with Serum Creatinine (Cr) > 2.0 mg/dL or estimated Glomerular Filtration Rate (eGFR) < 30 - Non-diabetic patients with Serum Creatinine (Cr) > 3.0 mg/dL or estimated Glomerular Filtration Rate (eGFR) < 15 - Patients with cervical spine disorders who are unable to undergo neck extension, which could potentially cause injury. - Patients with severe dyspnea caused by upper airway obstruction when in the supine position. - Patients with a history of prior thyroid, neck, or open-chest surgery (sternotomy/thoracotomy).
Design outcomes
Primary
| Measure | Time frame |
|---|---|
| The sensitivity and specificity of computed tomography of the neck in extended and neutral positions in patients with substernal goiter When the CT scan and surgery are performed number of case in each group | — |
Secondary
| Measure | Time frame |
|---|---|
| To evaluate the displacement distance of the thyroid gland during neck extension When neck CT scans are performed in both neutral and extension positions millimeter | — |
