Conditions

Participants are adult patients with suspected respiratory failure who require intubation and are randomized into either a 25 degree head elevation or supine sniffing position then receive preoxygenat

Inclusion criteria

Inclusion criteria: 1. Age more than 18 years 2. Respiratory failure that need for intubation by using Rapid Sequence Intubation method

Exclusion criteria

Exclusion criteria: 1. Trauma patient 2. Pregnancy 3. History of Difficult intubation 4. History of allergy to medications used for RSI 5. Predicted to have difficult intubation based on the LEMON assessment more than 1 criteria is exclusion 6. BMI more than 40 7. Cardiac arrest 8. Crash airway 9. Inability to make a decision within 10 minutes or clinical deterioration
