Inclusion criteria

Inclusion criteria: Participants will be enrolled as family dyads consisting of one caregiver and one care recipient. Both members of each dyad must meet all respective criteria. Caregivers inclusion criteria: Age 20 years or older; self identified as a primary or secondary caregiver for an older adult family member with chronic conditions; living in the same household with the care recipient or visiting at least 3 times per week; able to read, write, and communicate in Thai language; willing to participate in an 8 week program with weekly 90 minute sessions; available to attend sessions together with the care recipient; not currently participating in other chronic disease management research; able to provide written informed consent; residing in Pak Phli District, Nakhon Nayok Province, Thailand. Care recipients inclusion criteria: Age 50 years or older; physician diagnosed type 2 diabetes mellitus or essential hypertension; for diabetes, hemoglobin A1c 7.0 percent or higher or fasting blood glucose more than 130 mg per dL documented on two consecutive laboratory tests within the past 3 months; for hypertension, blood pressure 140 over 90 mmHg or higher documented on two consecutive measurements; currently receiving oral medication treatment for diabetes or hypertension and not insulin dependent; Mini Mental State Examination score 18 or higher; residing in Pak Phli District, Nakhon Nayok Province, Thailand for at least 6 months with intention to remain for the study duration; able to communicate verbally in Thai language; willing to participate in the 8 week program together with a family caregiver; able to provide written informed consent or assent with caregiver consent if mild cognitive limitations are present; not currently participating in other chronic disease management research studies.

Exclusion criteria

Exclusion criteria: Participants will be excluded if either member of the family dyad does not meet eligibility requirements or meets any exclusion criteria. Caregivers exclusion criteria: Age less than 20 years; unable to communicate in Thai language; unable to attend the program sessions with the care recipient; currently participating in other chronic disease management research studies; unable or unwilling to provide written informed consent; not residing in Pak Phli District, Nakhon Nayok Province, Thailand; severe physical or mental condition that prevents participation in the intervention. Care recipients exclusion criteria: Age less than 50 years; diagnosed with type 1 diabetes mellitus; currently using insulin therapy; severe cognitive impairment defined as Mini Mental State Examination score less than 18; severe hearing or vision impairment preventing participation; severe medical complications or unstable condition requiring hospitalization; inability to communicate in Thai language; unable or unwilling to provide informed consent or assent; currently participating in other chronic disease management research studies; planning to relocate outside Pak Phli District during the study period.
