Eligibility
Inclusion criteria
Inclusion criteria: 1) First-time subcortical ischemic stroke or subcortical hemorrhagic stroke occurring more than 6 months ago. 2) Age between 40 and 80 years 3) Fugl-Meyer Assessment of Upper Extremity score between 30 and 57, indicating moderate impairment. 4) Adequate cognitive function, defined as a Mini-Mental State Examination Thai version 2022 score of more than 23 for individuals with an education level higher than Grade 12, more than 18 for those with primary school education or below, and more than 15 for uneducated participants. 5) Able to sit independently for at least 2 hours without assistance. 6) Able to perform a reach-and-grasp (RTG) task, such as reaching and opening a hand sufficiently to grasp a 4 cm object and wrist extension 10 degrees. 7) Normal or corrected hearing and vision, to ensure full participation in instructions and task performance.
Exclusion criteria
Exclusion criteria: 1) Contraindications for Transcranial Magnetic Stimulation (TMS), such as seizures, as confirmed by a TMS screening questionnaire. 2) Have lesion(s) in other brain area(s) such as occipital, brainstem, cerebellar, and if brain damage was bilateral or of non-vascular origin. 3) Pain issues in two or more upper extremity joints (e.g., shoulder, elbow, wrist), as assessed by the Fugl-Meyer of the upper extremity section. 4) Present with other neurological disorders 5) Concurrent or recent participation in other brain stimulation protocols, including TMS or tDCS 6) Unstable vital signs.
