Inclusion criteria

Inclusion criteria: 1. Aged 60 years or older. 2. Diagnosed with respiratory failure. 3.Receiving mechanical ventilation via naso-endotracheal tube, oro-endotracheal tube, or tracheostomy tube. 4.Receiving mechanical ventilation for no more than 24 hours with no prior history of weaning during the current admission. 5.Fully conscious, with a Glasgow Coma Scale (GCS) score of E4VTM6. 6.No signs of delirium, as assessed and confirmed by the Confusion Assessment Method for the Intensive Care Unit (CAM-ICU) across all four features. 7.Normal cardiovascular and respiratory parameters defined as: RR between 10 and 30 per min, HR between 60 and 120 per min, BP between 90/60 mmHg and 180/110 mmHg, MAP greater than 65 mmHg, and O2sat greater than 90 percent. 8. Able to understand and communicate in the Thai language. 9. The patient is willing to participate in the study, and the attending physician approves their participation.

Exclusion criteria

Exclusion criteria: 1.Patients with a tracheostomy tube and a history of home ventilator use for more than 7 days. 2.Patients with neuromuscular diseases, including Myasthenia Gravis, Guillain-Barre syndrome, rheumatic diseases such as dermatomyositis and polymyositis, chronic inflammatory demyelinating polyneuropathy, and muscle atrophy. 3.Patients with rib fractures involving flail chest. 4. Patients with multiple organ failure. 5.Patients currently receiving sedatives, neuromuscular blocking agents, or inotropic drugs. 6.Patients who have previously experienced ventilator weaning combined with respiratory muscle training using elastic bands.
