Eligibility
Inclusion criteria
Inclusion criteria: 1.) Individuals aged 60 years old and above 2.) Residents of Baguio City, and have resided in the area for six (6) months and above 3.) Currently taking five (5) or more medications, whether, traditional or herbal remedies, or over-the-counter medications, liniments and balms 4.) Able to read and write in English, Tagalog, and/or Ilokano 5.) Demonstrates intact cognitive function, as evidenced by a Mini-Cog Assessment score ranging from 3 to 5 6.) Monthly attends regular scheduled check-ups at the health center
Exclusion criteria
Exclusion criteria: 1.) Older adults who are diagnosed with acute terminal illness requiring hospitalization and intensive care 2.) Currently participating in another clinical and research study that may affect medication adherence 3.) Exhibit conditions or impairments that may interfere with their ability to provide accurate medication information 4.) Residing in institutional settings (e.g., nursing homes and long-term facilities) where medication management is handled by a healthcare staff
