Inclusion criteria
Inclusion criteria: Inclusion Criteria 1. Aged 60 years or older, both male and female, with fragility hip fracture 2. Post-operative hip fracture patients discharged from the hospital within 1 week 3. Barthel ADL Index more than four points 4. The Six Items of Cognitive Function Test less than seven points 5. Language proficiency in Thai 6. Must have at least one family caregiver 7. Able to walk independently prior to the fall
Exclusion criteria
Exclusion criteria: 1. Have a history of severe physical or mental health conditions. 2. Caregiver of participants do not consent to participate in the study Withdrawal Criteria 1. Participants or caregiver request to withdraw from the study 2. Cannot complete all sessions of the program 3. Have health complication during the intervention 4. Failure to report health behaviors for more than four consecutive weeks.
