Inclusion criteria
Inclusion criteria: 1 : Male and female patients diagnosed with Type 2 Diabetes Mellitus (T2DM), aged 18 years or older. 2 Currently receiving continuous medical care and follow-up at Community Health Center 1 or Wat Bang Plee Yai Klang Community Medical Center, BangPlee Hospital. 3 Having a documented HbA1c level greater than or equal to 7.0% but less than or equal to 10.0% within the past 6 months. 4 : Patients who are currently not on insulin therapy or those who have declined the initiation of insulin treatment. 5 Proficient in reading, speaking, and listening to ensure effective participation and communication. 6 Willingness to provide informed consent and voluntarily participate in the research study.
Exclusion criteria
Exclusion criteria: 1 Moderately complex elderly mean those with more than 3 chronic conditions; example osteoarthritis, cancer, heart failure, depression, COPD, hypertension, urinary incontinence, CKD stage 3, myocardial infarction, stroke, or loss of Instrumental Activities of Daily Living , or mild to moderate cognitive impairment. 2 Highly complex elderly: Those requiring long-term care, those with end-stage chronic diseases example NYHA Class at least III, oxygen-dependent COPD, ESRD requiring dialysis, metastatic cancer), loss of basic Activities of Daily Living, or moderate to severe cognitive impairment. 3 Severe or Unstable Cardiovascular Conditions mean Including severe heart failure or unstable symptoms, and history of ischemic heart disease within the past 6 months. 4 Specific Types of Diabetes mean Diagnosis of diabetes before age 30 without clear features of Type 2 Diabetes (T2DM). 5 Strong family history suggestive of monogenic diabetes. 6 Suspected Latent Autoimmune Diabetes in Adults or Type 1 Diabetes Mellitus. 7 Pediatric and adolescent patients aged < 18 years. 8 Gestational diabetes. 9 Psychiatric Comorbidities mean Presence of clinical psychiatric disorders that may interfere with study participation or adherence.
