Inclusion criteria
Inclusion criteria: 1. Patients diagnosed with Ischemic Stroke at least three months since the initial diagnosis. Participants must have attended their two most recent follow-up appointments at the Chronic Disease Clinic, Yala Hospital, where office blood pressure was recorded by a registered nurse in the HosXP database. The recorded blood pressure must be more than or equal 140 mmHg (systolic) and/or more than or equal 90 mmHg. 2. normal cognitive function based on the TMSE. 3. physical ability must have a Modified Rankin Scale 2 out of 6. 4. Language proficiency in Thai. 5. ability to communicate via telephone and use an Android smartphone for two-way communication through the LINE application.
Exclusion criteria
Exclusion criteria: Exclusion Criteria 1. Have a history of secondary hypertension diagnosis 2. Have a history of severe mental health conditions such as depression or schizophrenia, who are currently undergoing treatment. 2. Have a history of end-of-life stage 3. Caregiver of participants do not consent to participate in the study. Withdrawal Criteria 1. Participants or caregiver requests to withdraw from the study 2. Cannot complete all sessions of the program 3. Have health complication during the intervention, such as hypertensive crisis more than or equal 180/120 mmHg, recurrent stroke, acute coronary syndrome, or sudden cognitive decline. 4. failure to report health behaviors via the LINE application for more than four consecutive weeks.
