35 Years to 70 Years

Inclusion criteria

Inclusion criteria: 1. Patients aged 35 to 70 years 2. Diagnosed with chronic kidney disease stage 2 by a physician 3. Estimated glomerular filtration rate between 60 and 89 milliliters per minute per 1.73 square meters 4. No adjustment or change in medications related to chronic kidney disease including ACEI ARB diuretics antihypertensive drugs or antidiabetic drugs within the past two months 5. Patients with access to electronic devices including smart phone notebook or computer and able to use Line application 6. Able to speak read and write in Thai language

Exclusion criteria

Exclusion criteria: 1. Patients with chronic kidney disease with physical disability of upper or lower extremities 2. Patients unable to perform activities of daily living with ADL score less than 11 3. Patients who have previously participated in a self management program for delaying renal deterioration 4. Patients aged 60 years with cognitive impairment with Mini Cog score less than 3 5. Patients who consume alcohol or smoke 6. Patients with other comorbidity
