Inclusion criteria
Inclusion criteria: Inclusion criteria for patients 1.Critically ill surgical patients admitted to the Surgical Intensive Care Unit at Phra Pok Klao Hospital during the study period. 2.Aged 15 years or older. 3.Able to communicate in Thai sufficiently for clinical assessment and interaction. 4.Provide informed consent to participate in the study (written, verbal or non verbal, as approved by the ethics committee). Inclusion criteria for nurses 1.Registered nurses who are permanently working in the Surgical Intensive Care Unit at Phra Pok Klao Hospital during the implementation period of the guideline. 2.Directly involved in bedside care of critically ill surgical patients and in using the nursing practice guideline for delirium prevention. 3.Willing to participate in the study and complete the required assessment forms and questionnaires.
Exclusion criteria
Exclusion criteria: 1.Patients with acute delirium before admission to the ICU 2.Patients with chronic psychiatric disorders such as schizophrenia or bipolar disorder 3.Patients with dementia or chronic neurological diseases such as Alzheimer disease 4.Patients who are unable to communicate permanently 5.Patients with a continuous RASS score of -4 to -5 without any evaluable period for CAM ICU 6.Patients who die or are discharged before completing the follow up period 7.Patients or relatives who do not consent or request withdrawal from the study

| Nursing Guideline Adherence Rate Once per shift throughout the ICU stay during the post implementation period. Adherence is measured as the percentage of completed items on the nursing practice guideline checklist recorded by nurses.,Nurse Satisfaction with the Guideline At the end of the implementation period after nurses have used the guideline in routine practice. Nurse satisfaction is measured using a self administered Likert scale questionnaire after implementation. Mean scores and satisfaction levels are calculated. | — |
