Inclusion criteria

Inclusion criteria: Age 60 years or older Pain intensity score 3 or higher on the Visual Analog Scale Ability to understand and fully comply with the exercise training program Presence of pre existing balance impairment prior to study enrollment defined as a Berg Balance Scale score less than 52 indicating mild to moderate fall risk and a Timed Up and Go test time greater than 12 seconds Diagnosis of knee osteoarthritis according to the American College of Rheumatology criteria defined as knee pain plus at least three of the following clinical features Morning stiffness lasting less than 30 minutes Crepitus during knee joint movement Bony tenderness of the knee joint No palpable warmth over the knee joint

Exclusion criteria

Exclusion criteria: History of lower extremity surgery History of fracture or injury to the lower extremities within the past 6 months Diagnosis of inflammatory joint or muscle disorders such as rheumatoid arthritis or polymyalgia rheumatica Body mass index greater than 40 kg per square meter Neurological disorders such as multiple sclerosis or peripheral neuropathy Cardiovascular or vascular diseases including ischemic heart disease deep vein thrombosis or varicose veins Medical conditions limiting safe exercise participation including uncontrolled hypertension or cardiovascular disease History of medical treatment or physical therapy for the back within 1 week prior to study enrollment Regular exercise participation more than 3 times per week with cessation of exercise less than 4 weeks prior to study enrollment

Muscle strength baseline and 4 week Knee extensor and hip abductor muscle strength will be assessed using an isokinetic dynamometer (Biodex System 4 or Biodex System Pro 4 Computerized Dynamometer, Shirley, New York, USA)
