Interventions

Fentanyl NB 25 mcg plus NSS up to 4 ml maximum 50 mcg per dose Maximum 300 mcg per day and NSS 3 ml IV,Fentanyl IV 25 mcg maximum 50 mcg per dose maximum 200 mcg IV per hour and NSS 4 ml NB

Inclusion criteria

Inclusion criteria: 1.terminally ill patient (18-100 year) in emergency department 2. who declined life -sustaining treatment (CPR, intubation, Inotrope, Hemodyalysis) 3. present with dyspnea and RR more than 24

Exclusion criteria

Exclusion criteria: 1. terminally ill patient who require life -sustaining treatment 2. allergy in fentanyl 3. obstruction of nasal passage, Nasopharyngeal cancer, Paranasal sinus malignancies
