Patients with CKD stage IV-V, age 1-18, receiving treatment at Srinagarind hospital.

Inclusion criteria

Inclusion criteria: 1. Pediatric patients with chronic kidney disease stages 4 and 5, aged 1 to 18 years, who are receiving treatment at Srinagarind Hospital 2. The patient and their parent(s)/guardian(s) are willing to participate in the study.

Exclusion criteria

Exclusion criteria: 1. Patients with the following comorbid conditions: Severe infection Cancer Hepatitis, defined as having ALT or AST levels more than 2 times the upper normal limit (Srinagarind Hospital uses an upper normal limit of 40) 2. Receiving sodium valproate (which affects carnitine levels) 3. Currently receiving carnitine supplementation
