Inclusion criteria
Inclusion criteria: 1) Patients aged 18 years and older who has been diagnosed with Type2 DM, recorded using the International Classification of Diseases and Related Health 10th revision codes E110-E119. 2)Received continuous care every 3 months for 1 year.
Exclusion criteria
Exclusion criteria: 1.Patients with severe cancer, advanced-stage cancer, or cancer currently under active treatment 2.Patients with frailty 3.Patients with end-stage chronic diseases, including chronic kidney disease, heart failure, severe chronic obstructive pulmonary disease (COPD), chronic lung disease, and dementia. 4.Pregnant women 5.Foreigners who cannot communicate in Thai 6.Patients with discontinuous follow-up during the study period or whose laboratory tests were not performed according to the scheduled intervals 7.Patients with incomplete data according to the case record form
