Inclusion criteria

Inclusion criteria: 1.Age more18 years 2. Receiving vancomycin 5 consecutive days 3.Baseline serum creatinine more 1.5 mg/dl or eGFR more 60 4.Normal baseline magnesium level

Exclusion criteria

Exclusion criteria: 1.CKD stage 4 5 2.Current magnesium supplementation or magnesium-containing drugs 3.Concomitant nephrotoxic drugs (gamphotericin B cisplatin) 4.Pregnancy or lactation
