the patients diagnoesd with primary pterygium who have surgical indications at Uthaithani Hospital. pterygium

Interventions

Tetracaine group : instill 0.5% Tetracaine drops (1 drop every 5 minutes for 3 times ) and instill artificial tear gel 0.5 ml , for 5 minutes . Re administer Tetracaine drops and artificial tear gel

Inclusion criteria

Inclusion criteria: 1. patients aged 20-75 years 2. patients diagnosed with pterygium at Uthaithani hospital. 3. patient who have surgical indications Indications for Surgery. 1. reduce vision 2.obstruct sight 3.frequent irritation 4. limits eye movement 5.cosmetic concern

Exclusion criteria

Exclusion criteria: 1. recurrent pterygium 2. bi head pterygium 3. patient with known allergy to lidocaine or tetracaine 4. dementia , mental instability 5. patient who are on regular pain medication

level of pain during the initial application of the topical anesthetic agent 1 minute Numerical Rating Scale,number of additional tetracaine eye drops required as pain during the surgery 30 minutes number eyedrop,corneal fluorescein staining 2 minutes Oxford scale,patient satisfaction 5 minute metric,complication related to use topical anesthetic agent 2 weeks complication of topical anesthetic agent
