Inclusion criteria
Inclusion criteria: 1. Aged eighteen years or older. - For participants aged 60 years or older, a Mini-Cog assessment score of at least 3 were additionally required. 2. On stable standard HF medications and diuretics for at least one month. 3. Diagnosed with stable HF with NYHA functional class I or II. 4. No hospitalization due to HF within the past month. 5. Stable vital signs (systolic blood pressure at least ninety millimeters of mercury, diastolic blood pressure at least sixty millimeters of mercury, and heart rate less than one hundred beats per minute) 6. Able to walk independently. 7. Under ongoing care and follow-up by a physician at the HF Clinic or the General Medicine Clinic, with physician approval to participate in the study. 8. Able to listen, read, and write in Thai. 9. Own and use an Android smartphone with internet access.
Exclusion criteria
Exclusion criteria: 1. History of major cardiovascular events within the past 3 months, such as acute myocardial infarction (AMI), stroke, unstable angina, or arrhythmia requiring emergency treatment. 2. History of cardiovascular procedures within the past 3 months, including: a) Coronary interventions, such as percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) or b) Cardiac device implantation or activation, such as a permanent pacemaker (PPM) or implantable cardioverter defibrillator (ICD) or c) Valvular interventions, including surgical aortic valve replacement (SAVR), transcatheter aortic valve replacement (TAVR), or transcatheter edge-to-edge repair (TEER) 3. Presence of medical conditions that limit mobility or the ability to engage in PA, such as being under palliative care, having stage IV cancer, end-stage renal disease [estimated glomerular filtration rate (eGFR) less than 15 ml per minute per 1.73 square meter], or severe osteoporosis. 4. Planned major surgery or elective medical intervention within the next three months. 5. Current participation in a CR program or any other PA promotion program.
