Inclusion criteria
Inclusion criteria: 1. Aged 60 years and above; 2. diagnosed with CHF and NYHA functional class I, II, or III for more than six months by a cardiologist; 3. not having other severe complications from their health problems that prevent full participation in all program processes; 4. having no sensory cognitive problems; 5. maintaining a good cognitive state; 6. having inadequate health literacy; 7. being able to care for oneself; and 8. being willing to participate in the program until the study was finished.
Exclusion criteria
Exclusion criteria: 1) being hospitalized during any of the program sessions and 2) not being willing to continue the program.
