Inclusion criteria

Inclusion criteria: 1. First diagnosed as stroke (ischemic or hemorrhagic) 2. less than 3 months after stroke onset 3. Experimental group: Living in catchable area within 30 minutes by car from Siriraj hospital 4. Barthel index score < 15 5. stable in medical and neurological condition 6. Able to understand Thai language and follow commands (Verbal and gesture) 7. Able to follow up at 4 weeks 8. Caregiver can support patient during physical therapist provide intervention at home. 9. Caregiver consent to participate

Exclusion criteria

Exclusion criteria: 1. Were diagnosed with unstable diseases or pathologies e.g., cancer, serious cardiovascular/pulmonary disease, uncontrolled hypertension 2. Having neurological or psychological condition that disturb learning 3. Cognitive impairment (TMSE < 20) 4. Aphasia 5. Dependency before stroke
