Inclusion criteria
Inclusion criteria: Quantitative phase 1. Previously diagnosed with a history of mild to moderate hypertension (BP 140/90-179/100 mmHg) for at least one year and recorded in the database by the Primary Care Unit, Siriraj Hospital 2. Determined as having uncontrolled hypertension according to the criteria of BP (BP 140/90- 179/100 mmHg) for two consecutive measurements among Thais with hypertension who received both pharmacological and non-pharmacological treatments at the Primary Care Unit, Siriraj Hospital 3. Having the intention to change in self-management behaviors according to the stage of change questionnaire 4. Able to be accessed by smartphone 5. Able to communicate in the Thai language Qualitative phase 6. Aged 45-79 years old who have BP lower than 140/90 mmHg after participating in self-management combined with digital health interventions. 7. Aged 45-79 years old who have BP greater than 140/90 mmHg after participating in self-management combined with digital health interventions.
Exclusion criteria
Exclusion criteria: Quantitative phase 1. Having any severe complications such as cardiovascular, cerebrovascular, and end-stage renal disease 2. Having inadequate digital health literacy as measured by the eHealth Literacy Scale (eHEALS). The cut-off points of the lower 26 indicate inadequate digital health literacy (Richtering et., 2017) 3. Being cognitive impairment as measured by the Thai version of Mini Cog. A cut-off point of 2 from 5 refers to cognitive impairment (Trongsakul, Lambert, Clark, Wongpakaran, & Cross, 2015) 4. Individuals with moderate to severe dependency or complete inability to perform basic activities of daily living, as assessed by the Thai version of the Barthel Index (range score 0-11) (Jittapankul, Kamolratanakul, & Ebrahim, 1994), will be excluded. In addition, those who are unable to use a telephone or manage their own medications, as evaluated using the Thai version of the Lawton Instrumental Activities of Daily Living Scale (Phanasathit, 2017) 5.
