Inclusion criteria

Inclusion criteria: - Age 60 years or older - FRAIL score more than 3 out of 5 - Deconditioning with ADL decline more than 2 points compared to pre-admission - ADL score less than 15 within 3 months - National Health Security Office coverage - Eligible for Siriraj home visit service after hospital discharge

Exclusion criteria

Exclusion criteria: Advanced dementia Cancer or palliative patients with life expectancy less than 6 months Long term care patients Patients living in the same household as another study participant
