Inclusion criteria
Inclusion criteria: 1. Retired Royal Thai Army personnel aged 60 and older. 2. Having hypertension and receiving medical care. 3. Need assistance from non-medical home care providers. 4. Residing in the authorized area of Fort Sunpasitthiprasong Hospital, Ubon Ratchathani province. 5. Willing to participate in the study and provide written informed consent.
Exclusion criteria
Exclusion criteria: 1. Bedridden patients. 2. End-stage renal disease with renal replacement therapy or eGFR <30ml/min/1.73sq-m. 3. A history of myocardial infarction six months previously. 4. A history of heart failure; New York Heart Association (NYHA) class III and over. 5. Participating in other clinical controlled trials.
