Inclusion criteria
Inclusion criteria: 1. population aged 35 - 70 years 2. Diagnosed with A1C >7 and/or HT >/= 130/80 mmHg 3. using smartphone 4. connecting to internet 5. communicate in Thai
Exclusion criteria
Exclusion criteria: 1. Diagnosed with psychosis 2. lived in the area less than 1 year
