Inclusion criteria
Inclusion criteria: 1. Age 20 years and older, 2. Diagnosed with at least hypertension or dyslipidemia or obesity, 3. At mild to moderate risk for vascular complications including uncontrolled diabetes, uncontrolled hypertension, hyperlipidemia, a family history of vascular or heart disease. A nurse will assess these risks using the Thai CV risk score, while a family doctor will confirm eligibility by reviewing the medical history of patients, 4. Able to use digital devices and read Thai, 5. Able to provide informed consent.
Exclusion criteria
Exclusion criteria: 1. Acute medical condition requiring hospitalization, 2. Cognitive impairment or inability to participate, 3. Already enrolled in a similar program, 4. Recent diagnosed with strokes, hypertensive heart disease, ischemic heart disease, Heart Failure, Acute Myocardial Infraction, Psychosis disorder
