Conditions

The main points of care in COPD patients were self-care efficiency, no exacerbation, improved quality of life, and clinical outcomes. Chronic obstructive pulmonary disease, self- care behaviors, quality of life and clinical outcomes

Interventions

The intervention group will receive a transitional care program. Duration 3 months.,The Control group will receive a usual care. Duration 3 months.

Inclusion criteria

Inclusion criteria: 1. Dx. COPD 2. V/S normal no exacerbation in 24 hr. 3. Six Cognitive Impairment Test score 0 - 7

Exclusion criteria

Exclusion criteria: Severe exacerbation
