Inclusion criteria

Inclusion criteria: 1. Diagnosis of STEMI and able to receive PPCI within 12 hours after the onset of symptoms. 2. Coronary angiography shows occluded or reduced flow or obstructive coronary artery stenosis > 50% with evidence of thrombus at culprit artery

Exclusion criteria

Exclusion criteria: 1. severe hemodynamic disturbance before PCI (cardiogenic shock SCAI stage equal or more than D, need for intra-aortic balloon pump, malignant arrhythmia) 2. Life expectancy less than 1 year 3. Bypass graft or stent thrombosis or in stent restenosis lesion 4. Pregnancy 5. Contraindications to heparin or antiplatelet therapy 6. Severe tortuous, or angulated coronary anatomy of the study vessel that in the opinion of the investigator would result in suboptimal imaging or excessive risk of complication from placement of an imaging catheter 7. Inadequate image quality for angio-IMR computation (poor image quality, excessive vessel overlap)
