Inclusion criteria

Inclusion criteria: 1. ASA physical status I-III 2. Undergoing elective surgery requiring general anesthesia with endotracheal intubation 3. At least one predictor of difficult intubation - BMI 35 and above - Mallampati class III or IV - Thyromental distance less than 6.5 cm - Neck flexion-extension less than 90 degrees - Mouth opening less than 4 cm - Inability to protrude lower jaw beyond upper - History of difficult intubation

Exclusion criteria

Exclusion criteria: 1. Patients requiring alternative intubation techniques (e.g., awake fiberoptic) 2. Cognitive or communication impairment 3. Inability to understand Thai language 4. Coagulopathy 5. Airway, oral, or cervical spine abnormality (e.g., trauma, mass, infection) 6. Pregnancy 7. Withdrawal of consent
