Inclusion criteria
Inclusion criteria: 1. Aged 40 years or older. 2. The severity level of stage 2. 3. Experienced at least one acute exacerbation in the past year. 4. Fully conscious, no cognitive impairment, as assessed by the Mini Mental State Examination; MMSE-Thai 2002 for individuals aged 60 years or older. 5. Has a caregiver, family member, or a significant person important to the patient. 6.able to understand and communicate in Thai without any hearing or vision problems. and 7. First-time participant in the advance care planning program and has never completed an advance directive for end-of-life care.
Exclusion criteria
Exclusion criteria: Has an illness related to cancer in various organs.
