Inclusion criteria
Inclusion criteria: 1. Adults aged 18 years or older who have been diagnosed with, or have a documented history of, ischemic stroke (ICD-10: I63) and were admitted to the inpatient ward at Sam Chuk Hospital. 2. A score of 12 or higher on the Modified Barthel Index for Activities of Daily Living (ADL), indicating at least moderate functional independence. 3. A score of greater than 23 on the Thai Mental State Examination (TMSE), reflecting normal cognitive functioning. 4.Ability to read, speak, write, and communicate effectively in the Thai language. 5.Access to a communication device compatible with video conferencing platforms and a stable internet connection. 6.Willingness to participate in the study and provision of written informed consent.
Exclusion criteria
Exclusion criteria: 1.Diagnosed with cognitive, perceptual, or memory-related disorders, such as Alzheimer disease or psychiatric conditions. 2.Demonstrating clinical deterioration to the extent that they are no longer able to perform basic self-care activities.
