Inclusion criteria
Inclusion criteria: 1) Adults aged 30-59 years. 2) Diagnosed with hypertension by a physician at least 6 months ago and prescribed at least one antihypertensive medication. 3) Uncontrolled hypertension, with systolic/diastolic levels between 140/90 and 179/109 mmHg in the past month. 4) Controllable comorbidities (based on clinical parameters from medical history), such as diabetes mellitus (FBS less than 250 mg/dl and no symptoms of diabetic crisis). 5) Being able to communicate, read, and write in Thai
Exclusion criteria
Exclusion criteria: Exclusion criteria 1) No access to a smartphone with internet connectivity. 2) Severe complications such as vision and hearing loss, musculoskeletal problems, acute coronary syndrome or stroke within the past 6 weeks, renal failure (serum creatinine greater than 3 mg/dl), altered consciousness, or terminal illness. 3) Severe hypertension with systolic blood pressure 180 mmHg or higher and or diastolic blood pressure 110 mmHg or higher. 4) Pregnancy, lactation, or planning pregnancy during the trial. 5) History of back surgery, or coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI) within the past 6 months. 6) Non-ambulatory patients. Termination criteria: 1) Incomplete participation in all program sessions. 2) Emigration or death during the program. 3) Severe illness or critical condition requiring hospitalization during program activities. Withdrawal criteria: Participants who request to withdraw due to scheduling conflicts or in ability to participate in program activities.
