Eligibility
Inclusion criteria
Inclusion criteria: 1. Participants aged 18 years and older were diagnosed with heart failure by a physician., 2. Patients with heart failure severity and activity capability are classified as New York Heart Association functional class II or III., 3. The patients symptoms have been stable for at least 24 hours, and the patient has stable vital signs and no dyspnea., 4. Participants could communicate in Thai and had no cognitive impairments. Patients over 60 years old were assessed for cognitive impairment using the Six-item Cognitive Impairment Test total score had to be less than 8 points., 6. Participants could communicate via telephone and Line applications., 7. Consent was required to participate in the research.
Exclusion criteria
Exclusion criteria: 1. Patients whose condition worsened or who died during hospitalization or the research process., 2. Patients who were unable to be contacted after discharge from the hospital., 3. Patients who withdrew from the study during the research period., 4. Patients with heart failure who received other care programs during the research process.
