Inclusion criteria
Inclusion criteria: 1. The patients aged 65 years and older who understand and can communicate in Thai. 2. The patients at the general medicine outpatient clinic, male medical ward, and female medical ward of Maharaj Nakorn Chiang Mai Hospital. 3. Patients at the outpatient and inpatient departments of Bangkok Hospital Phuket.
Exclusion criteria
Exclusion criteria: 1.Patients with dementia 2.Patients with altered consciousness that interferes with assessment 3.Patients with severe hearing or vision impairment that hinders assessment 4.Patients with unstable symptoms/vital signs or on mechanical ventilation, making assessment difficult 5.Patients under contact precautions for infectious disease monitoring
