Inclusion criteria

Inclusion criteria: 1. Thai Adults aged 60 and older. 2. Having any noncommunicable disease and receiving medical care. 3. Need assistance from non-medical home care providers. 4. Residing in the authorized area of Baan Pae Health Promoting Hospital or Wat Sophon Health Center, Mueang Rayong District, Rayong Province. 5. Willing to participate in the study and provide written informed consent.

Exclusion criteria

Exclusion criteria: 1. Bedridden patients. 2. End-stage renal disease with renal replacement therapy or eGFR <30ml/min/1.73sq-m. 3. A history of myocardial infarction six months previously. 4. A history of heart failure; New York Heart Association (NYHA) class III and over. 5. Participating in other clinical controlled trials.
