Inclusion criteria
Inclusion criteria: 1. Age > 18 years 2.Received treatment at the medical ICU or intermediate ward (7NW, 8IK, and 9IC) 3. Under mechanical ventilator for at least 48 hours 4 .Received a ventilator with PAV+ mode (Puritan-Bennett 980 ventilator (Nellcor Puritan Bennett LLC, Gosport, UK Gosport, UK) 5. The patient is ready to wean off the ventilator -The patient is capable of initiating an inspiratory effort and has a normal level of consciousness -Uses a fraction of inspired oxygen less than 0.4 (FiO2 < 0.4) PEEP <= 8 cm H2O -No excessive tracheal secretions (requires more than normal sputum suction) -The patient or relatives consent to participate in the research project.
Exclusion criteria
Exclusion criteria: 1. Patients with unstable vital signs: 2. Mean blood pressure < 65 mmHg 3.Unstable arrhythmia (HR>150 or < 40 bpm) 4 .Patients with oxygen saturation < 90% 5. Patients with neurological abnormalities that affect breathing 6 . Patients treated with supportive care 7 . Patients who have a tracheostomy tube 8 .Patient on PSV mode with pressure support <= 5 cmH2O and awaiting extubation 9. Patients or relatives refuse to participate in research or request to withdraw from the research
