Conditions

This study focuses on patients with schizophrenia diagnosed according to ICD-10 criteria (codes F20-F29). The health conditions and problems studied include: Psychiatric symptoms - measured by Brief Psychiatric Rating Scale (BPRS) Depression - measured by 9Q assessment Suicide risk - measured by 8Q assessment Medication adherence and drug-related problems Hospital/Emergency Room readmission rates Functional level Quality of life - measured by EQ-5D-5L questionnaire and Visual Analog Scal

Interventions

Home visits by a multidisciplinary team consisting of: Frequency and Duration: 3 visits, 1 month apart Each visit lasts 45 minutes Service Provider Team: Preventive Medicine Doctor, Community Ment

Inclusion criteria

Inclusion criteria: 1.Patients diagnosed with schizophrenia according to ICD-10 (codes F20-F29) 2.Registered for treatment at Erawan Hospital, Loei Province 3.Age 20 years or older 4.BPRS score less than 36 points 5.Depression score (9Q) less than 12 points 6.Suicide risk assessment score (8Q) less than 8 points 7.Able to communicate and answer questions 8.Willing to participate in the study

Exclusion criteria

Exclusion criteria: 1.Drug addiction 2.Pregnant or breastfeeding women 3.Referred to other healthcare facilities 4.Relocation during study period 5.Unable to contact during months 1-2 before home visits 6.BPRS score more than 36 points 7.9Q score more than 12 points 8.8Q score more than 8 points
