Inclusion criteria
Inclusion criteria: 1. T2DM and stage 1 and stage 2 CKD (eGFR over and equal 60 ml permin per 1.73 m sqeare and urine albumin creatinine ratio [UACR] over and equal 30 mg/g). 2. Being diagnosised confirmed at least twice within the last 3 months. 3 Aged over and equal 40 years. 4. Presence of at least one risk factor for CKD progression as follows. 4.1 SBP over and equal 140 mmHg. 4.2 DBP over and equal 90 mmHg. 4.3 HbA1c over and equal 7 percent, in the past 3 months. 4.4 Absence of cognitive impairment, as indicated by a MINICOGTM Thai version score over 3, for participants aged over and equal 60 years.
Exclusion criteria
Exclusion criteria: Presence of severe or acute complications related to T2DM and CKD as follows 1. Heart attack 2. Acute stroke 3. Ischemic heart disease.
