Inclusion criteria

Inclusion criteria: 1. Age over 18 years 2. Current use of a ventilator with pressure triggering 3. a decision by the attending physician to extubate within 24 hours 4. adequate neurologic status Glasgow Coma Scale equal or more than 11 points 5. hemodynamic stability systolic blood pressure more than 90 mmHg 6. having received the minimal dose of vasoactive drugs 7. heart rate equal or less then 140 per min 8. SpO2 more than 90 percent while using fraction of inspired oxygen equal or less than 0.4 9. positive end-expiratory pressure equal or less then 8 cmH2O 10. respiratory rate equal or less then 35 per min 11. effective cough evaluated by physician in charge

Exclusion criteria

Exclusion criteria: 1. tracheostomy patients 2. progressive neuromuscular disease 3. on home mechanical ventilation 4. recent myocardial infraction within one month 5. decompensated heart failure with ejection fraction less than 30 percent 6. upper airway obstruction 7. excessive secretion 8. having received sedation or a neuromuscular blocking agent within 24 hours 9. arrhythmia
