Inclusion criteria
Inclusion criteria: 1. All documented hospital errors that occurred within the selected hospitals of Urmia County during the years 2022-2023 were included. 2. Errors had to have been officially recorded in the hospital management system to be considered for the study. 3. Both preventable and non-preventable errors were included in the analysis. 4. Errors involving all types of healthcare professionals (physicians, nurses, paramedics, etc.) were included.
Exclusion criteria
Exclusion criteria: 1. Errors that occurred outside of the selected hospitals or outside of the specified time frame were excluded. 2. Incidents that were not formally classified as hospital errors in the hospital's documentation system were excluded. 3. Errors that resulted in no harm to the patient and were deemed insignificant by the hospital's quality control team were excluded. 4. Duplicate error reports were identified and excluded to avoid double-counting.
